Florida Region Indoor Club Academy for New Club Leaders

The Indoor Club Academy for new club leaders will be hosted at the Florida Region office this year! Please contact the Florida Region staff at (352) 742-0080 during normal office hours (M-F, 9a-5p) if you have any questions.

IMPORTANT ANNOUNCEMENT – ONE Director of each New Club is REQUIRED to attend the 2025 Indoor Club Academy.
All other Club Directors are exempt from attending the 2025 Indoor Club Academy.
​Note: Clubs that are required to attend and do not will incur a $500 fine and will not be recognized as a USAV club in good standing until the fine has been paid in full. Clubs will be required to attend an Indoor Club Academy within the next 365 days to remain in good standing for the following season. Clubs that do not attend an indoor Club Academy the second time around will not be recognized as a USAV club in good standing indefinitely. This is a standing mandate from the Florida Region Board of Directors.

Junior Coach in Training Waiver Request Form​ is required for Junior ages 16 and 17 who are interested in coaching
USA Volleyball has a ZERO TOLERANCE for abuse and misconduct. This includes not only on-court safety, but also off-court safety in any part of USA Volleyball’s programs.
This form is required to be compliant with Florida Law, effective July 1, 2021, this medical treatment consent form is required to be completed and signed by a parent/legal guardian for any minor athlete to receive emergency and non‐ emergency medical care at a USA Volleyball sanctioned event/activity in Florida. Athletes should carry multiple copies of this form in their backpacks to all USAV sanctioned practices, camps, clinics, tryouts and events. The signed form will need to be presented to any medical provider before care may be administered.
This form will no longer be required. However, the Florida Region recommends that clubs do collect the form from every athlete.
Due to increased participation of youth/junior athletes relocating to Florida from an outside federation, including Puerto Rico (PUR), the Florida Region has prepared this document to ensure that Florida Region clubs are following FIVB/USAV policy on athlete eligibility. Note: FIVB restricts athletes from being affiliated with more than one National Volleyball Federation at any given time.
Youth/junior athletes must meet the below requirements to be official members of the Florida Region of USA Volleyball:
The Florida Region may request any/all of the below required documents as proof of Florida Region residency for an athlete coming from any other federation, including PUR:
  • Current Florida Driver’s License
  • Signed lease agreement or proof of mortgage
  • Utility or phone bill with address in FL Region
  • Any other government issued document as requested
  • Most recent report card from a Florida school
  • Enrollment verification in a Florida school at the time a Florida Region membership is purchased
Note: Legal guardian may also provide documentation above but must also include legal documentation proving guardianship for junior athlete.
Any youth/junior athlete that is currently attending school and playing club in/for another federation is not eligible to become a member of the Florida Region.
If it is found that the athlete has purchased a USAV membership but is unable to provide documents requested above, their USAV membership will be placed on Region Hold and they will be unable to participate in any USAV sanctioned events. No refunds will be available for these memberships.
If an ineligible youth/junior athlete participates in a USAV sanctioned event in the Florida Region, the athlete and club will be subject to sanctions & penalties as outlined in the USAV Championship Manual.

Event Sanction Process

All Florida Region of USA Volleyball sanctioned events are required to follow these policies and procedures. Please review the following information carefully.
This document includes the various policies, guides, checklists, and resources you may need to run a successful event.
This policy outlines the specific requirements of TDs and ODs when operating a USA Volleyball sanctioned event in the Florida Region.
Please utilize this list before, during, and after an event to ensure that you are completing all USAV event requirements.

It is NOT necessary to submit both applications. Please read the information carefully to determine which application is best for your organization.
If you wish to register your club with the Florida Region of USAV, please complete this application prior to completing the event sanction form. If you do NOT wish to register a club, please submit the Event Affiliate Application below. More information about club requirements can be found here.
If you are not currently associated with a Florida Region club OR if your club will not be the main sponsoring organization for your event, please submit this application prior to completing the event sanction form.
*Please note that both of these are annual applications and are valid for the current season only (each USAV season runs from September-August).

Please submit an Event Sanction Application for each event (tournament, scrimmage, clinic, etc.) you are hosting. If you are running a league, please submit one Application for each section of the league (i.e., Summer Beach League, Fall Indoor League, etc).
Please do not submit your application until your Club Application or Affiliate application has been approved.
Note: This application is to be submitted at least 30 days prior to the event. After submission, please allow 7-10 business days for processing.
