Medical Consent Form
This form is requried to be compliant with Florida Law, effective July 1, 2021, this medical treatment consent form is required to be completed and signed by a parent/legal guardian for any minor athlete to receive emergency and non‐ emergency medical care at a USA Volleyball sanctioned event/activity in Florida. Athletes should carry multiple copies of this form in their backpacks to all USAV sanctioned practices, camps, clinics, tryouts and events. The signed form will need to be presented to any medical provider before care may be administered.
Medical Release Form
This form will no longer be required. However, the Florida Region recommends that clubs do collect the form from every athlete.

C.O.P.S. Report Form
The C.O.P.S. (Coaches, Officials, Players, and Spectators) form allows individuals in the Florida Region to submit a COMPLIMENT or a COMPLAINT regarding Coaches, Officials, Players, or Spectators (COPS) at a Florida Region/USAV sanctioned event.
Club Invitation and Affiliaton
- Club Affiliation
When purchasing a membership, members cannot select a club at checkout. Club invitations must be sent by the club.
- Athlete Commitment
During membership purchase, by accepting a club invitation, athletes are committing to that club for the entire season.
- After competing in a regional or national qualifier/bid event, transfers or releases must follow the transfer/release rules in the Girls’ or Boys’ Junior National manuals. Please refer to the appropriate manual for details.
SafeSport Training
Turning 18 this season? You will be required to complete SafeSport training, your SportsEngine profile will notify you. The SafeSport Core Course will be assigned to your profile and must be completed before you can be added to a USAV roster.
Junior Officiating Courses
Jr. Officiating Courses that an athlete is required to take will be automatically uploaded into their SportsEngine Profile.
